Telehealth delivery of comprehensive medication management (CMM) and other clinical services by pharmacy type: a pre- and post-COVID-19 pandemic experience analysis.
To determine telehealth use, an online questionnaire was administered to pharmacists from 27 pharmacies, differentiated into three pharmacy types: independently owned, integrated into clinical settings, and those operating within retail chains. An additional analysis was performed to determine if telehealth implementation of CMM services led to an improvement, no change, or a decline in care quality for patient subgroups, such as those with diabetes, individuals with low incomes, and those aged 65 or older.
Telehealth adoption soared among independent and clinically-affiliated pharmacies during the pandemic, while retail chains remained unchanged. Connectivity resources for telehealth services were scarce; yet, an increase in usage was observed for the first two pharmacy types. Telehealth CMM's effectiveness during the pandemic was highlighted by pharmacists in both independent (63%) and integrated (89%) pharmacies, enabling access to patients they would otherwise not have reached. Pharmacies and pharmacists found telehealth to be a functional and agreeable avenue for implementing CMM.
CMM via telehealth is now a familiar and desired path forward for pharmacists and pharmacies, even as the pandemic eases. Image-based analyses of neural activity have been shown to pinpoint cognitive deficits in individuals with a history of childhood trauma. The study's objective was to evaluate potential differences in executive function, employing functional near-infrared spectroscopy (fNIRS), between participants who reported experiencing childhood physical, emotional, or sexual abuse (n = 37) and a comparison group (n = 47) performing cognitive tasks. The child abuse group exhibited a statistically significant rise in both the rate and number of commission errors on the Conners CPT test, a difference that was apparent when contrasted with the control group's performance. The Wisconsin Card Sorting Test (WCST) underscored a statistically significant decrease in oxyhemoglobin (oxy-Hb) levels within the left rostral prefrontal cortex of participants in the child abuse group, when measured against the no-abuse group. During the OSPAN and Connors CPT, a comparable, though not meaningful, decrease in oxy-Hb concentration was observed in the right dorsolateral prefrontal cortex (dlPFC) of the child abuse group. The arrival of an African dwarf frog (Hymenochirus curtipes) colony at an animal research facility coincided with a surge in morbidity and mortality within the population. Mortality was observed among animals that were present on arrival, or animals deteriorated soon after. Subsequently, additional animals displayed clinical signs of lethargy, weight loss, and a lack of interest in food during the ensuing three weeks. Multifocal areas of hyperemia appeared on the limbs, inguinal, and axillary regions of some affected animals, with the ventral abdomen displaying mottled tan discoloration. Histological examination of the lesions confirmed a diagnosis of generalized septicemia, characterized by widespread inflammation, including granulomatous meningitis, otitis media, peritonitis (coelomitis), myocarditis, pericarditis, nephritis, pneumonia, and arthritis. Gram staining revealed rod-shaped, gram-negative bacteria, unencumbered and found both within the tissues and inside macrophages. Analysis of coelomic swabs revealed a moderate to substantial quantity of Elizabethkingia miricola. Water tested from tanks where affected animals were kept demonstrated elevated levels of nitrites and ammonia, concurrently with the presence of Citrobacter, Aeromonas, Pseudomonas, and Staphylococcus species. In this randomized controlled pilot trial, the internet-based, passive psychoeducational intervention, “Free From Abuse,” was evaluated for its potential to enhance healthy relationships in young adults. Among participants aged 18 to 24 years, a random assignment procedure categorized them into an intervention treatment group (n=71) and a placebo control condition (n=77). The intervention group showed a greater increase in the identification of abusive behavior and a decreased acceptance of domestic violence myths in comparison to the control group, observed immediately post-intervention and one week later. Preliminary evidence from this study suggests that brief, internet-based passive psychoeducation might be beneficial for fostering healthy relationships in young adults.
